Junior Consent Forms

The safety and wellbeing of our Juniors is paramount, the SafeGolf process is in place so that participants can thrive in a safe environment and have an enjoyable and positive experience when playing golf.

Prior to playing in a competition, county match or organsied training by NLCGA all Juniors must completed the following two documents and returned no later than the first day of activity:

1. Junior Player Profile and Parental Consent Form

2. Junior Code of Conduct Form

If these documents have not been returned then they must be completed on the day before any activity commences.

They only need to be completed once January-December 2021, however, any changes to personal details need to be forwarded to the County Junior Organiser Sherrell Chuter.
